Core operations for editable wiki databases (WIKIDB convention v3).
Every function here works on a plain context table and performs no
terminal I/O, so the whole module is testable against a temp database.

A context is bound to one collection. Entry operations touch that collection's content and meta keyspaces; media, events and sequences are database-wide.
